Download your own diaries

A very useful tool in building your self care programme is to keep a daily diary. This is also sometimes recommended in the Self Care Toolkit. Below you can download dedicated diary pages as pdfs for printing off at home.

They are designed so that in each day you record changes or events on the left and changes in symptoms on the right. By keeping such records over several weeks you can see patterns that are usually missed if you rely only on your memory.
